Emergencies

Don't use our website forms for an emergency. If you have a facial injury, swelling spreading toward your eye or neck, or trouble breathing or swallowing, call 911 or go to the nearest emergency room.

Appointment requests

Sending a request through our website doesn't book an appointment. Our team will call you during office hours to find a time.
